Current version has a different profile structure than the previous version. Who sets this structure? Does it come out of the box, or can it be administered?

Structure of profile comes predefined in the system. All the custom fields are kept in profile traits, or event properties and traits. Elasticsearch can dynamically add new fields to traits and properties.
